Loyalty Card Terms & Conditions

1. Eligibility for Reward: The referral reward applies only when the referred individual completes both a New Patient Consultation and a Report of Findings visit at Future Chiropractic. The referred individual must also mention their connection to the referring patient at the time of their initial appointment for the reward to be validated.

2. Reward Type and Redemption:

   • For patients on monthly pre-paid plans, the free adjustment will be applied to one of their scheduled visits in the next billing cycle, reducing the payment owed.
   • For patients paying per visit, the free adjustment can be redeemed on their next appointment after meeting the referral requirements.

3. Reward Application and Tracking:

   • Future Chiropractic staff must mark off each reward on the patient’s loyalty card, and the referral will also be recorded in our system to confirm eligibility.
   • Rewards will be honoured for each successful referral, with no limit on how often patients can earn a free adjustment.

4. Active Membership Requirement: This offer is only available to active members of Future Chiropractic who have had an adjustment within the last three months. Patients must maintain active status to redeem their rewards.

5. Non-Transferable: This reward is exclusive to the referring patient and cannot be transferred, gifted, or exchanged for cash or other services.

6. Exclusions and Restrictions:

   • The free adjustment cannot be combined with any other discount, promotion, or special offer.
   • The free adjustment will not be refunded if the patient cancels or reschedules the appointment outside our cancellation policy.

7. No Expiry: There is no expiry on the free adjustment reward, provided the referring patient remains an active member of the clinic.

8. Program Changes: Future Chiropractic reserves the right to modify or discontinue the loyalty card program, including any terms, conditions, or eligibility requirements, at any time without prior notice.

9. Referral Tracking and Verification: Referral rewards are tracked within our system, and it is the responsibility of the referring patient to ensure that the referral is correctly recorded by Future Chiropractic staff at the time of the new patient’s first appointment.

10. Privacy and Data Protection: All referral information will be handled in compliance with privacy laws, and patients’ personal information will not be shared outside of necessary business functions.

11. Limitations of Liability: Future Chiropractic is not responsible for any technical or system errors that may temporarily prevent the processing or tracking of referrals.
